Member Portal - Forgot Password

1. Go to the LIVE WELL website
2. Click the MEMBER SIGN IN link at the top right corner of the page
3. Click the forgot password link under the SIGN IN button
4. Enter your username or email to receive a password reset link
5. Follow the password reset instructions given in the email
